Confused About How Healthcare Reform Will Affect You? A New Guide Helps You Make Sense of It

The New York Times reports the Affordable Care Act was upheld by U.S. Supreme Court in June, signifiying what many hope will be the end of our reputation as the only rich country with large numbers of uninsured people.

It's a milestone in our history, but most of us are still in the dark about how exactly these new reforms will affect us. With open enrollment deadlines for insurance providers inching ever closer, now is a good time to finally get a grasp on what you can expect from new healthcare reform laws.?

Some of the most dramatically different benefits that consumers can expect include a ban on insurance companies canceling your plan if you get sick, or using your pre-existing condition as a reason to refuse you coverage. In addition, insurance companies are obligated to provide subscribers with better preventative care; services like cancer screenings and immunizations have to be made available at no cost by new private healthcare plans.

    Netanyahu to press for Iran 'red line' in U.N. speech

    NEW YORK (Reuters) - Israeli Prime Minister Benjamin Netanyahu will set out, in his speech at the United Nations on Thursday, an ultimatum for Iran to halt its disputed nuclear drive or risk coming under military attack, an Israeli official said.

    Netanyahu faces the world body after U.S. President Barack Obama disappointed some Israelis, in his own address to the annual assembly, by not calling for a deadline to be imposed on Tehran - though he did say time for diplomacy "is not unlimited".

    Israel sees a mortal threat in a nuclear-armed Iran and has long threatened to strike its arch-foe pre-emptively, agitating war-wary world powers as they pursue sanctions and negotiations.

    Complicating Netanyahu's strategy have been his testy relations with Obama as a U.S. election looms, and the reluctance of many Israelis to trigger a conflict with Iran, which denies that it is seeking to develop nuclear weapons and has pledged wide-ranging retaliation if attacked.

    "The prime minister will set a clear red line in his speech that will not contradict Obama's remarks. Obama said Iran won't have nuclear weapons. The prime minister will clarify the way in which Iran won't have nuclear arms," a senior Israeli official said en route to New York, without elaborating.

    Though he has not previously detailed when Israel might be willing to go to war, Netanyahu has said Iran could have enough low-enriched uranium by early 2013 to refine to a high level of fissile purity for a first nuclear device [ID:nL5E8KG2P5].

    Israel worries that this final step, if taken, could happen too quickly or quietly to be prevented.

    Iran has said it has no plans to enrich uranium beyond the 20 percent purity required to run a reactor producing medical isotopes. That level, however, brings raw uranium exponentially closer to the 90 percent enrichment required for bomb fuel.

    Though reputed to have the Middle East's sole nuclear arsenal, Israel would be hard-put to deliver lasting damage to Iran's remote facilities using its conventional forces, or to handle a multi-front war.

    WORK TOGETHER

    Netanyahu's public calls for a U.S. ultimatum have deepened acrimony with Obama, a Democrat accused by his Republican rivals of being soft on the Jewish state's security. That has stirred American accusations of Israeli meddling in the November presidential elections - something denied by Netanyahu.

    "The prime minister will say that Israel and the United States can work together to achieve their common goal," said the Israeli official on condition of anonymity.

    Netanyahu, who heads a broad-based, conservative coalition government, departed for New York on Wednesday saying he would take the U.N. podium for an Israel "united in the goal of preventing Iran from achieving nuclear weaponry".

    But surveys show that most Israelis - apparently swayed by the open dissent of several senior national-security figures - would oppose launching unilateral strikes on Iran, given the risk of alienating Washington and of provoking clashes with Tehran's Islamist militant allies in Lebanon and Gaza.

    A poll published by the liberal Haaretz newspaper on Thursday found that 50 percent of Israelis feared for the survival of their country, should there be a conflict.

    Iranian President Mahmoud Ahmadinejad, in his speech to the General Assembly on Wednesday, said Iran was under threat of military action from "uncivilized Zionists," a clear reference to Israel. Earlier this week, Ahmadinejad said that Israel would eventually be "eliminated."

    Haaretz also ran excerpts from a leaked Foreign Ministry report that sanctions had caused greater damage to Iran's economy than anticipated by Israel.

    The findings, confirmed to Reuters by an Israeli official, could undermine any attempt by Netanyahu to argue that the military alternative must be considered imminently.

    Israeli opposition leader Shaul Mofaz criticized Netanyahu for sparring with Obama and voiced confidence in U.S. resolve.

    "I am convinced that the United States, the president of the United States, is determined to prevent Iran going nuclear," Mofaz told Israel's Army Radio.

    Even within Netanyahu's coalition there have been misgivings about the pitch of disagreement with the United States.

    Danny Ayalon, deputy to Foreign Minister Avigdor Lieberman, described Obama's Iran remarks at the United Nations as "important, albeit measured".

    Speaking on Israel Radio, Ayalon said the Netanyahu government and Obama administration were in discreet contacts and approaching agreement on setting limits for Iran.

    Live?from the Built Green Conference 2012. Session 1 | Green ...

    Coming to you live this morning from the Built Green Conference in gorgeous Mukilteo. We are at the Rosehill Community Center above the ferry terminal.

    The structure this year is breakout sessions bookending a keynote speech at lunch. There also is an expo where we can meet vendors and other stakeholder sin energy efficiency and green homes. It is hard to decide which session to go to. I picked the one that best matches my personal interests and professional role as a real estate broker.

    Interestingly enough, I was an early participant in the Community Power Works program, before it reached its current iteration of providing a $95 energy assessment. A volunteer came to my home and talked about energy efficiency and how to cut your personal energy use. Low hanging fruit (one of the easy. low cost things you can do) included replacing all of our existing light bulbs with CFL bulbs. This was about two years ago. I had not changed out the light bulbs from when we bought the townhome as new construction in 2007. It is a 3-Star Built Green Certified home in the Built Green Certified Community of High Point in West Seattle. Besides energy efficiency, changing to a lower usage bulb makes your home more comfortable. All of those incandescent bulbs put out a lot of heat.

    There are a number of measuring tools that are being utilized to show the results of the Community Power Works retrofits. Over 500 homes have been completed, with over 240 in the pipeline now. The gal si to reduce energy use by at least 15%. Only one home has not met that goa, anf there are some ?audacious? homes that are saving 50-60% in energy use on their utility bills.

    Building science
    Rather than write all new specs, they use Department of Commerce specifications for building performance.
    Reducing air leakage in a home (insulation)- leaky recessed lighting fixtures increases heat loss/gain, and can cause ice dam problems.
    Most often performed corrective action taken in homes in the program:
    Air sealing
    Attic insulation
    Wall insulation
    Floor insulation
    uct sealing
    Duct insulation

    Measurement used to be in carbon savings. Most people couldn?t understand it so it was changed to energy savings. Quality assurance has to be evaluate frequently so course corrections can be made. Programs like this leverage current training to bolster both new and existing workers and create not just jobs but quality companies.

    Who are the customers/
    Income between $50-$150k. There is a significant drop off in the higher income brackets, perhaps these flks don?t consider government programs for tem. There also is a low number of households with people of color. Perhaps there is a language barrier or translation problem that needs to be increased. More outreach to lower income communities. There are cultural liasons speaking about 12 languages
    33% of households have children in the home
    Most homes were built before 1959.

    Average cost of upgrade $14,500
    Average CPW rebate: $2,600. (18%)
    Average utility rebates: $700 (5%)
    Customers with CPW loans: 25%

    Washigton State University (WSU) is a big participant in the program, helping with surveys of homeowners after they have participated in the program.

    What is the future of Community Power Works?
    Under negotiations now withs stakeholders trying to get interest gauged from utilities and assessing market need. By Jan or Feb 2013 there may be an extension to the program (but no new money coming in).

    PST: Why give Klose praise for his handball?

    UPDATE:?As many of you fine commenters pointed out, I was totally wrong about the time and situation. I apologize for that mistake. But I do think most of the points remain, with the exception of the paragraph that begins ?Furthermore?? ?Thanks for getting me honest, everyone.

    ORIGINAL POST: In a match against Napoli,?substitute?Miroslav Klose entered with his Lazio club down 3-0. He scored four minutes later. Except he didn?t.

    The German international appeared to put the ball past Morgan De Sanctis, but he hit it with his hand. Chaos ensued, Klose admitted his misdeed, and play continued without the goal counting. The final scoreline: 3-0.

    The internet is all ablaze with people praising Klose for his heroic performance. I?m not picking on the New York Times, but here?s a link that sums up the effusiveness.

    So, okay, I get it: Klose did the honorable thing. But? did he really? It?s pretty clear he intentionally hit the ball with his hand. In my mind, admitting you cheated after the fact is not the same as not cheating in the first place. I credit him for saying what happened ? that is admirable ? but I?d give him more credit for not hitting the ball with his hand in the first place. (Of course, then we wouldn?t be having this discussion.)

    Furthermore, it came late in a match that was already lost. I wonder if Klose would have owned up so quickly if it were the winning goal. I?m not saying he wouldn?t; I?m just saying pressure does funny things to people even when they have every intention of acting honorably. (Hey Mr. Henry.)

    While we?re here, the whole ?players calling themselves on infractions the refs missed? thing bothers me, too. It undermines the official?s authority: ?You can?t do your job, so I?ll do it for you.? That seems like a slippery slope.

    This is, I admit, a harsh reading of the situation. Klose deserves some praise for his actions; I?m just not sure we should hand out the ?Honorable Footballer of the Year? award quite yet.

    Melatonin and exercise work against Alzheimer's in mice

    ScienceDaily (Sep. 26, 2012) ? The combination of two neuroprotective therapies, voluntary physical exercise, and the daily intake of melatonin has been shown to have a synergistic effect against brain deterioration in rodents with three different mutations of Alzheimer's disease.

    A study carried out by a group of researchers from the Barcelona Biomedical Research Institute (IIBB), in collaboration with the University of Granada and the Autonomous University of Barcelona, shows the combined effect of neuroprotective therapies against Alzheimer's in mice.

    Daily voluntary exercise and daily intake of melatonin, both of which are known for the effects they have in regulating circadian rhythm, show a synergistic effect against brain deterioration in the 3xTg-AD mouse, which has three mutations of Alzheimer's disease.

    "For years we have known that the combination of different anti-aging therapies such as physical exercise, a Mediterranean diet, and not smoking adds years to one's life," Coral Sanfeliu, from the IIBB, explained. "Now it seems that melatonin, the sleep hormone, also has important anti-aging effects."

    The experts analysed the combined effect of sport and melatonin in 3xTg-AD mice which were experiencing an initial phase of Alzheimer's and presented learning difficulties and changes in behaviour such as anxiety and apathy.

    The mice were divided into one control group and three other groups which would undergo different treatments: exercise -unrestricted use of a running wheel-, melatonin -a dose equivalent to 10 mg per kg of body weight-, and a combination of melatonin and voluntary physical exercise. In addition, a reference group of mice were included which presented no mutations of the disease.

    "After six months, the state of the mice undergoing treatment was closer to that of the mice with no mutations than to their own initial pathological state. From this we can say that the disease has significantly regressed," Sanfeliu states.

    The results, which were published in the journal Neurobiology of Aging, show a general improvement in behaviour, learning, and memory with the three treatments.

    These procedures also protected the brain tissue from oxidative stress and provided good levels of protection from excesses of amyloid beta peptide and hyperphosphorylated TAU protein caused by the mutations. In the case of the mitochondria, the combined effect resulted in an increase in the analysed indicators of improved performance which were not observed independently.

    Treatment not easily transferable to humans

    "Transferring treatments which are effective in animals to human patients is not always consistent, given that in humans the disease develops over several years, so that when memory loss begins to surface, the brain is already very deteriorated," the IIBB expert points out.

    However, several clinical studies have found signs of physical and mental benefits in sufferers of Alzheimer's resulting from both treatments. The authors maintain that, until an effective pharmacological treatment is found, adopting healthy living habits is essential for reducing the risk of the disease appearing, as well as reducing the severity of its effects.

    The melatonin debate

    The use of melatonin, a hormone synthesized from the neurotransmitter serotonin, has positive effects which can be used for treating humans. With the approval of melatonin as a medication in the European Union in 2007, clinical testing on this molecule has been increasing. It has advocates as well as detractors, and the scientific evidence has not yet been able to unite the differing views.

    According to the Natural Medicines Comprehensive Database, melatonin is probably effective in sleeping disorders in children with autism and mental retardation and in blind people; and possibly effective in case of jet-lag, sunburns and preoperative anxiety.

    "However, other studies which use melatonin as medication show its high level of effectiveness," Dar?o Acu?a-Castroviejo explained. He has been studying melatonin for several years at the Health Sciences Technology Park of the University of Granada.

    The expert points out that international consensus already exists, promoted by the British Association for Psychopharmacology -also published in the Journal of Psychopharmacology in 2010-, which has melatonin as the first choice treatment for insomnia in patients above the age of 55. This consensus is now being transferred to cases of insomnia in children.

    Its use in treating neurodegenerative diseases is acquiring increasing scientific support in lateral amyotrophic sclerosis, in Alzheimer's, and Duchenne muscular dystrophy.

    "Even though many more studies and clinical tests are still required to assess the doses of melatonin which will be effective for a wide range of diseases, the antioxidant and anti-inflammatory properties of melatonin mean that its use is highly recommended for diseases which feature oxidative stress and inflammation," Acu?a-Castroviejo states.

    This is the case for diseases such as epilepsy, chronic fatigue, fibromyalgia, and even the aging process itself, where data is available pointing to the benefits of melatonin, though said data is not definitive.

    BENNINGTON, Neb. (AP) ? Duane Braesch's cornfields are prime evidence of how unforgiving the elements have been for him and so many others across the Midwest this summer. To demonstrate the hardship, the 79-year-old Nebraska farmer let The Associated Press show the world what he's weathered during the worst U.S. drought in decades.

    Using a camera powered by solar panels and mounted on a pole on a mound overlooking Braesch's cornfield near Omaha, AP photographer Nati Harnik chronicled the wilting effects of extreme heat over August and much of September that turned Braesch's crop from a vibrant emerald to a sickly yellow.

    Snapping a picture every 10 minutes, the camera was shrouded in plastic to shield it from rainfall. But significant rain came just twice in 59 days, helping explain why Braesch and his son ultimately reaped just half of what they would typically expect in a harvest.

    Yet during a summer in which many other farmers simply declared their crops a complete bust and simply knocked them down for feed to livestock, Braesch figures "we're just pretty lucky we got what we got."

    "You wouldn't think there'd be anything out there," he said. "It's amazing it's as good as it is."

    That's considering what the time-lapse photos show: Clouds pass over Braesch's farmland day after day, at times teasing the grower with the look of storms but offering little relief other than a rare shower and, more often, meaningless spits of water. The stalks turn discolored, so brittle the leaves scrape each other like sandpaper.

    Juvenile murderer ruling reopens 'traumatic wounds'

    Nineteen years ago, Bobbi Jamriska's younger sister was found murdered in a Pennsylvania schoolyard. As Jamriska grieved, one thing brought her solace: When a court found her sister's 16-year-old ex-boyfriend guilty and sentenced him to life in prison without parole.

    "When you get up every day, you think about what happened, but at least you know that there was that one constant, that life-without-parole was going to make sure that you never had to relive that part of it," said Jamriska, 40, who lives in Pittsburgh.

    But three months ago, the Supreme Court struck down mandatory life-without-parole sentences for juveniles as cruel and unusual punishment.?While the June 25 ruling wasn't necessarily designed to be applied retroactively, some youth advocates are trying to use it to free so-called "juvenile lifers," setting?off a series of battles over what to do with the approximately 2,100 convicted murderers who were handed mandatory life-without-parole sentences for acts committed as youths.

    For victims' families, the decision has had huge emotional, and in some cases, legal implications.

    "After the [Supreme Court] ruling, everyone felt like they were reliving the trial phase and their loved ones' murder," said Jamriska, who traveled to Washington, D.C., with other victims' families to protest the ruling.

    She is part of a support group called the National Organization of Victims of Juvenile Lifers.

    "There were a lot of families who didn't have any idea that this was even possible," she said. "For them, it was literally one day business as usual, and then the next day, on the news, their whole world got turned upside down."

    Pennsylvania, where Jamriska lives, has the biggest concentration of juveniles serving mandatory life sentences -- about 480 of them, the oldest of whom was convicted almost 60 years ago and is now in his 70s, according to the Juvenile Law Center. Earlier this month, the state Supreme Court in Pennsylvania began hearing arguments for why some of the lifers there should be paroled, including the ex-boyfriend who killed Jamriska's sister in 1993.

    No one in the legal system told Jamriska that the parole arguments involved her sister's killer. She found out from a reporter's voicemail about three weeks after the Supreme Court ruling that lawyers were trying to get parole for him.

    Jamriska was stunned, but she said a lack of communication is somewhat understandable.

    "There never was a contingency for if this person who was sent to life in prison with no parole is suddenly able to get out," she said. "The DA's office isn't really staffed to manage that influx of appeals and those victims who are trying to get information -- I don't blame them."

    The state Supreme Court has put the arguments on hold and didn't give a timeline for a ruling. The Pennsylvania legislature still needs to come up with an appropriate alternative punishment for minors going forward.?

    "The sentencing scheme in Pennsylvania currently provides that for any individual, juvenile or adult, convicted of first or second-degree homicide must either receive a sentence of death or a sentence of life without parole. For juveniles, that mandatory sentence of life without parole has been declared unconstitutional," Marsha Levick,?deputy director and chief counsel at the Juvenile Law Center, said. "We think the courts should look to the next most severe sentence that is statutorily available in the state. Here, that means a sentence for third-degree murder, where you have a maximum sentence of 40 years."

    Levick suspects lawyers in other states will argue for that too. Since the Supreme Court ruling, North Carolina has passed a law replacing the mandatory life without prison sentence with a 25 years to life sentence; California's governor is currently evaluating a law that sets up two different schemes where parole eligibility comes in at either 15 or 25 years to life, Levick said. In all, 28 states still allow?mandatory life-without-parole sentences for minors, a situation that will have to change.

    "States can still impose life without parole," she said. "They just can't make it the only sentence available."

    As some juvenile advocates try to undo sentences that have already been imposed, Jennifer Bishop-Jenkins, 54, president of the National Organization of Victims of Juvenile Lifers, worries about the families of their victims.

    "Whenever you reopen traumatic wounds or you're triggering a retraumatization, you're talking about something that is going to affect people's work, their sleep, their health, their marriages -- everything," she said.

    Victims can only rely on each other for support, Jenkins said.

    14 years old: Too young for life in prison?

    "They don't register for victim notification and they don't monitor what's happening, and then you get these reactions like what we've been getting in our organization," Bishop-Jenkins said. "We've been trying very hard to find people to let them know that this multi-billion dollar campaign to free their loved ones' killers is going on and they're just shocked."

    There are potential legal issues too: Bishop-Jenkins' pregnant sister and brother-in-law were murdered in their home in Winnetka,?Ill., 22 years ago. It was Bishop-Jenkins' father who found their bodies; his testimony served as crucial evidence in the initial trial. Eight years ago, her father died of cancer. She says the judge from the first trial has also died.

    "My father was the best eyewitness to the carnage of the crime scene. We didn't videotape him talking about the crime," Bishop-Jenkins said. "We didn't get the transcript of what the judge said at the sentencing hearing where he gave this speech about if anybody deserved life without parole, he did."

    She now fears she and her mom, 83, could have to face her sister's killer in sentencing hearings in court. And while she doubts he will be granted parole, she said she worries lawyers may try again every couple of years.

    What Happens When the Amish Go to Prison?

    Sixteen members of a breakaway Amish sect were convicted of hate crimes in Ohio on Thursday. The jury found them guilty of violently shaving the hair and beards of disfavored members of the community, and they now face up to 20 years in prison. How do American prisons accommodate the old-fashioned religious practices of Amish people?

    They don?t, for the most part. State and federal prison systems don?t have special regulations for the tiny number of Amish serving extended sentences, many of them for sex offenses. The Amish reject most modern technology and dress. In prison, however, their cells have electric lighting and climate control, they wear orange jumpsuits, and they are transported between the prison and the courthouse in vans, just like other inmates. Prison administrators generally require prisoners to keep their beards trimmed, but wardens sometimes allow exceptions for religious reasons.

    U.S. prisons are required to accommodate religious beliefs under the Religious Land Use and Institutionalized Persons Act, but inmates? religious freedoms are tempered by security and cost concerns. Striking this balance has proven challenging for judges. In the case of facial hair, for example, courts have struck down outright beard bans, but agreed with wardens that excessively long or unkempt facial hair prevents guards from quickly identifying inmates. Prisoners who require special meals have also met with mixed responses. An inmate can demand a halal meal, but it is often just the prison?s vegetarian meal, because the facility refuses to pay the added expense of halal butchering.

    Amish inmates who are forced by the state to use electricity or wear brightly colored clothing aren?t violating their religious beliefs in quite the same way as, for example, an Orthodox Jew who is fed non-kosher food. Amish are allowed to use modern technology under certain circumstances: Amish farmers often rent lighted stalls at farmers? markets, and members of liberal communities hire drivers to transport them to supermarkets for weekly shopping. The prohibition has more to do with excessive engagement with modern technology. Amish people are not supposed to wire their homes or own automobiles, because those behaviors might distract them from the values of community, humility, and simplicity, and violate what they view as one of the Bible?s most central commandments: ?Be not conformed to this world.?

    Complying with the church commandments isn?t such a problem for many Amish inmates, because the church usually excommunicates members who commit crimes of violence or depravity before they enter prison. The ex-convict may be able to rejoin the church after release if he confesses and apologizes.

    County jails, which hold detainees for shorter periods, are more accommodating than long-term prisons to Amish detainees. In the 1950s and ?60s, many Amish fathers served short stints in these facilities for refusing to send their children to public high schools. (The U.S. Supreme Court freed them from this obligation in 1972.) Their jailers typically allowed them to wear traditional conservative clothing and long beards. While jails aren?t as casual as they once were, they still show some flexibility. Last year, a jail in Mayfield, Ky., special-ordered nine dark-colored jumpsuits for a group of Amish men sentenced to a few days for refusing to place reflective triangles on their horse-drawn buggies.
